تعیین غلظت کل تیروکسین (T4) در تشخیص آزمایشگاهی برای تمایز بین شرایط یوتیروئید ، پرکاری تیروئید و کم کاری تیروئید از اهمیت برخوردار است.از انجا که بخش عمده ای از T4 تام به پروتئین های انتقال دهنده متصل شده است (گلوبولین متصل شونده به تیروکسین: TBG ، پرآلبومین و آلبومین)،مقدار T4 تام فقط زمانی اطلاعات صحیحی را فراهم می کند که ظرفیت اتصالی تیروکسین )(thyroxine-binding capacity (TBC)( در سرم طبیعی باشد. هورمون های آزاد تیروئید با هورمون های متصل به پروتئین های حامل تعادل دارند.
ازمون TBC یا T-uptake امکان اندازه گیری جایگاه های در دسترس اتصالی تیروکسین را فراهم می کند. تعیین شاخص تیروکسین آزاد (FTI) از مقدار تام T4 و شاخص اتصال تیروکسین (یعنی نتیجه تعیین میزان T-Uptake ) تغییرات پروتئین های حامل هورمون تیروئید و سطح تیروکسین را در نظر می گیرد.

این آزمایش را نمی توان در بیمارانی که تحت درمان با عوامل کاهنده لیپید حاوی D-T4 هستند ، استفاده کرد. اگر قرار است عملکرد تیروئید در چنین بیمارانی بررسی شود ، ابتدا باید درمان برای مدت 4 تا 6 هفته قطع شود تا وضعیت فیزیولوژیکی دوباره برقرار شود.
