تشخیص منونوکلئوزعفونی زمانیکه تست های غربالگری منفی باشند ولی علائم مشکوک به عفونت با ویروس EBV وجود داشته باشد.

ویروس اپشتاین بار (EBV)عضوی از خانواده ویروس های هرپس و عامل ایجاد منونوکلئوز عفونی می باشد.
تشخیص عفونت های ناشی از ویروس EBV معمولا مشکل است. زیرا ویروس EBV در محیط های کشت سلولی معمولی در آزمایشگاه رشد نمی کند. اغلب عفونت های ناشی از ویروس EBV معمولا از طریق شناسایی آنتی بادی های هتروفیل (به روش آگلوتیناسیون لاتکس (Mono test)) در سرم بیمار تشخیص داده می شود. این نوع از آنتی بادی ها در حدود 3 هفته پس از شروع بیماری در سرم بیمار قابل شناسایی می باشند و پس از یک هفته سریعا شروع به کاهش می کند. این نوع از آنتی بادی ها در 10% افراد بالغ و اغلب کودکان تشکیل نمی شود.
اغلب موارد عفونت های هتروفیل منفی می توانند ناشی از عفونت با ویروس سایتومگال (cmv) باشند امّا نیمی از موارد هتروفیل منفی نیز می تواند در برخی از عفونت های ناشی از ویروس EBV مشاهده شود.
در مواردیکه شک به عفونت با ویروس EBV می باشد امّا آنتی بادی هتروفیل منفی است ارزیابی پروفایل اختصاصی آنتی بادی های علیه ویروس EBV می تواند در تشخیص بسیار کمک کننده باشد.
این آنتی بادی ها شامل موارد زیر می باشد :
- آنتی بادی علیه آنتی ژن کپسید ویروس
EBV viral capsid antigen (VCA)
VCA IgM , VCA IgG
- و هم چنین آنتی بادی علیه آنتی ژن هسته ای ویروس EBV
EBV nuclear antigen (EBNA)
عفونت با ویروس EBV معمولا در ابتدای زندگی اتفاق می افتد. برای مدت چند هفته تا چند ماه پس از شروع حاد عفونت این ویروس می تواند از طریق ترشحات بزاقی حاوی ویروس به فرد دیگر منتقل شود.
در میان علائم بالینی ناشی از عفونت با ویروسEBV، منونوکلئوز عفونی جزء شایعترین علائم بالینی می باشد. از اختلالات دیگر ناشی از عفونت با این ویروس می توان به لنفوم بورکیت و کارسینومای نازوفارنژیال اشاره کرد.
این ویروس هم چنین می تواند باعث اختلالات لنفوپرولیفراتیو در بیمارانی که پیوند مغزاستخوان یا کلیه انجام داده اند و یا افراد مبتلا به AIDS شود.

حضور آنتی بادی از جنس IgM علیه VCA نشان دهندۀ عفونت اخیر اولیه با ویروس EBV است و حضور آنتی بادی IgG علیه VCA نشان دهندۀ عفونت قبلی با این ویروس است.
آنتی بادی علیه EBNA ، 8-6 هفته پس از عفونت اولیه در بدن ظاهر می شود و تا آخر عمر در بدن باقی می ماند.
بیش از 90% از افراد بالغ طبیعی دارای آنتی بادی از جنس IgG علیه VCA و EBNA می باشند. تعداد کمی از بیماران که به ویروس EBV آلوده شده اند قادر به تولید آنتی بادی علیه EBNA نمی باشند (حدود 5-10 درصد از بیماران).

در صورتیکه فقط آنتی بادی از جنس IgG علیه VCA مثبت باشد ولی آنتی بادی IgM علیه VCA و آنتی بادی IgG علیه EBNA منفی شود، نشان دهندۀ عفونت با ویروس EBV می باشد امّا زمان عفونت قابل پیش بینی نمی باشد.
- نمونه هایی که در فازهای ابتدایی بیماری جمع آوری شده اند ممکن است از لحاظ حضور آنتی بادی علیه EBV منفی باشند و نمونه گیری بایستی 1-2 هفته بعد تکرار شود.
- نتایج آزمایشات بایستی در کنار علائم بالینی بیمار و نتایج سایر آزمایشات بیمار تفسیر شود.
- زمان ظهور آنتی بادی های مختلف علیه آنتی ژن های VCA و EBNAممکن است در افراد مختلف و براساس روش های آزمایشگاهی مختلف ، متفاوت باشد.
- این روش برای ارزیابی آنتی بادی در افراد با نقص ایمنی، نوزادان و نمونه خون بندناف استانداردسازی نشده است.
- این روش جهت تشخیص کارسینومای نازوفارنژیال، لنفوم بورکیت و یا سایر لنفوم های همراه با ویروس EBV مناسب نمی باشد.
- آنتی بادی IgG علیه VCA ممکن است با آنتی بادی IgM علیه این Ag رقابت کند و باعث نتایج منفی کاذب شود.
- فاکتور روماتوئید، در حضور آنتی بادی IgG اختصاصی ممکن است باعث نتایج مثبت کاذب شود.
- ارزیابی حضور آنتی بادی از جنس IgM علیه VCA نبایستی به عنوان تست غربالگری در جمعیت عمومی مورد استفاده قرار بگیرد.
ارزش پیش گویی نتایج مثبت و منفی بستگی به حضور یا عدم حضور علائم بالینی دارد و انجام آزمایش فقط بایستی در افرادیکه دارای علائم بالینی مشکوک هستند، صورت گیرد.
